Contract

50% of training fee is due at time of signing this Contract; Balance due upon pick up. Dog will NOT be released until ALL fees have been paid in full.

I understand that by agreeing to payment I have committed to the full length of the training program as stated in the Description of Services above. I understand that I am responsible for payment in full regardless of whether I choose to complete the training program.

Cancelation Policy: Non-refundable, no-transferable. Owner has the right to pick-up dog before above noted departure date and in so doing agrees to forfeit unused training fees and/or pay remaining balance in full upon pick-up.

If the dog has been in the care of Isok9 for more than 30 days of scheduled pick-up date with no payment, and owner/agent cannot be contacted, Isok9 will consider the dog to be abandoned. A certified letter will be sent requiring payment within 10 day period, the dog will be taken to an animal shelter.

Owner/agent agrees to provide any special care supplies (medications, heartworm prevention, flea tick prevention, special diet, supplements, etc.) at their own expense and authorize the administration of said special care supplies. Owner/agent agrees to pay for any ancillary products and services requested.

Owner/ agent accepts responsibility for any damages the above dog may cause through malicious, aggressive or responsibility and/or liability for injuries, conditions, illness, escape or loss to the above named dog that may occur during boarding, training, transportation or competition because supplies were not provided by owner/ agent Isok9 is also noting that this is a contract that is an AS-IS contract.
